The Saint Joseph's Family Health Center provides high-quality primary care medical services for our patients by personalizing health care that meets their needs. The Family Health Center is proud to be recognized by the National Committee on Quality Assurance as a Patient Centered Medical Home (PCMH). A PCMH is an approach to primary care that puts you at the center of your care.

We use teamwork to help deliver care that is personalized, comprehensive, coordinated and more convenient for patients--the way primary care should be. If you need access to other services or a specialist, the care team will guide you through the process from beginning to end. Our Care Teams consist of a provider, nursing staff and support staff who arrange all of their patients' treatment needs. Care Team members, including board-certified family physicians, also coordinate with other providers or services to meet your total health care needs. Our board-certified family physicians are trained to care for people of all ages and health conditions. We support you and your family through the stages of development from newborn through geriatrics.

The most important member of your health care team will always be with you.

What to Expect as a Patient

Access to clinical advice available 24 hours a day, 365 days a year. You can call (914) 375-3200 and a member of your team or a provider will return your call.

• Access to clinical advice when you need it.
• Convenient appointments, including same day, evening and Saturday, if required.
• Excellent communication from the Care Team regarding tests results, procedures and medications that relate to your care and treatment.
• Patient education to help you fully understand your health care needs and take an active part in self-management.
• Access to our Patient Portal to securely view your results, visit summary and much more.

We accept most commercial insurance programs as well as Medicare, Medicaid, Family Health Plus and Child Health Plus. For patients without insurance, we can provide you information about the New York State of Health insurance programs or help you apply for charity care based on your family income and household size.
